The Capacitance Manometer (CM) product line and associated pressure sensing element assembly (sensor, a CM component) require an export license for export or re-export to certain countries and/or end users as indicated in the US Export Administration Regulations (EAR Part 738 Supplement 1).

If Brooks Instrument will be the Exporter of Record for this shipment, we will require the following information in order to submit an application for the export license. A. Completion of the non-nuclear-statement on the letterhead of the ultimate consignee as follows
1a. We the Consignee <Full Name and Address of ultimate consignee / End User>
1b. Have ordered from (the Exporter) <Full Name and Address of intermediate consignee / purchaser from Brooks Instrument>
2. Quantity of each model or component ordered
3. Specify proposed end use of the item
4. Signed by: Printed Name & Title of Signature Authority & Signature of Signature Authority
B. Completion of the Transaction-Screening-Form. Please provide concise / specific description of the application and how the item will be used.
C. Single submission purchase order OR if customer requirements meet the BIS blanket export license criteria, Brooks can apply for a blanket license valid for 12-18 months. In this case, please provide the forecasted quantity, product description and unit price.
Export licenses take 4-6 weeks for approval from submittal date of complete information.
